REGISTRATION: COURSES FOR CERTIFICATION

STATE REQUIREMENT: In order to finalize your registration, the following forms must be completed, signed. and returned by mail:

• Enrollment Agreement

• Receipt of Enrollment Policies

• Previous Education Record

• Copy of High School Diploma, GED Certificate OR College Transcripts

• Receipt of Acknowledgement of School Tour

This is the LAST CERTIFICATION CLASS

I must have a minimum of 5 students signed up for the whole course (Basic, Intermediate, Advanced, and Marketing) by Sunday, Aug. 27^th in order to offer this course.

Due to the fact that money may need to be refunded to you if enrollment doesn’t meet the minimum requirements (5 students),
